Metformin HCl
Indications/Uses
Type 2 DM in adult particularly in overwt patients when dietary management & exercise alone does not result in adequate glycaemic control. May be used as monotherapy or in combination w/ other oral antidiabetic agents or insulin.
Dosage/Direction for Use
Monotherapy & combination w/ other oral antidiabetic agents 500-mg tab Initially 1 tab once daily. Increase dose by increments of 500 mg every 10-15 days. Max: 2,000 mg (4 tab) once daily. 750-mg tab 1 tab once daily. Max: 2 tab once daily. Adjust all doses based on blood glucose measurements after 10-15 days. Patient already treated w/ metformin Start w/ same daily dose of metformin IR tab. Transfer from another oral antidiabetic agents: Discontinue the other agent & initiate at dose indicated previously. Combination w/ insulin Initially 500 mg once daily, adjust dose of insulin based on blood glucose measurements.
Administration
Should be taken with food: Take w/ the evening meal. Swallow whole, do not chew/crush.
Contraindications
Hypersensitivity. Diabetic ketoacidosis & pre-coma. Dehydration, severe infection, shock, intravascular administration of iodinated contrast agents. Cardiac or resp failure, recent MI, shock. Acute alcohol intoxication, alcoholism. Renal failure or dysfunction. Hepatic insufficiency. Lactation.
Special Precautions
Lactic acidosis may occur in patients w/ significant renal failure. Monitor renal function regularly. Discontinue use prior to or during iodinated contrast agent administration; 48 hr before elective surgery or if metabolic acidosis is suspected. Increased risk of hypoglycaemia w/ concomitant use of insulin or other oral antidiabetics. Pregnancy. Childn. Elderly.
Adverse Reactions
Nausea, vomiting, diarrhoea, abdominal pain, loss of appetite; taste disturbance.
Drug Interactions
Alcohol, iodinated contrast agents, glucocorticoids, β2-agonists, diuretics, ACE inhibitors.
